Thank you for your interest in the Vermont Caregiver Survey.
Family members and friends provide critical care to patients in our healthcare system. But, because much of this care happens outside the clinic, we don't always know how best to support these caregivers.
Our goal for this survey is to learn more about caregivers in Vermont and surrounding states so that we can improve the ways we support these important members of our healthcare community. We want to know how who is providing care, what kind of care they provide, and what kinds of support they want.
It should take about 20 minutes to complete this survey. At the end of the survey, we provide a link to a list of resources that may be useful to you as a caregiver.
Participation in this survey is voluntary. Some responses are required, such as your name and contact information. Your name and contact information will not be shared without your permission (see below). You have the option to skip any other questions. However, the more information you provide, the better we will understand your experience in providing care.
All data is stored on secure University servers. When we create reports, we will take out your name and contact information, then group your responses with others to make sure you cannot be identified.
